Bilingual (Spanish) Geriatric Support Services Coordinator (GSSC) A GSSC is an Elder Services Social Worker who assesses, develops care plans and monitors SCO (Senior Care Organization) enrollee services status and participates on the Interdisciplinary Team. The Interdisciplinary Team works together to assure effective coordination and delivery of care in the home. Qualifications BA/BS in social work, human services or related field required; experience with Elders in a Health Care setting preferred; Associate’s Degree with significant relevant work experience may be substituted for portion of degree. Strong written, verbal, computer and skills necessary to work successfully in an interdisci-plinary team required. Bilingual ability (Spanish) required. Full Time: 37.5 hours per week Salary: $18.60 per hour
Care Manager Work as part of a dynamic, selfmanaging team with community elder clients in a large, well-established, private, non-profit organization. Duties: Assessment, service planning, providing information and referral to elders living in the community; and working within an interdisciplinary team. Qualifications: BA/BS in social work, human services or related field required; experience with elders preferred; Strong written, verbal, and computer skills required. Bilingual ability (Spanish or Khmer) a plus. Full Time: 37.5 hours per week Salary: $16.96 per hour
